Privacy notice

What the form collects, why, and who may see it. Effective October 28, 2026.

Who we are

Acetaminophen Claims Center operates this website at tylenollawsuit.health. We are not Kenvue, Johnson & Johnson, Kimberly-Clark, or a seller of acetaminophen.

What we collect

If you submit the form, we collect your name, phone number, email address, ZIP code, state, and the screening answers: who the claim is for, authority to sign, legal custody, the child's age, the diagnosis, tobacco, alcohol, illicit drugs, or SSRI use in pregnancy, how many times acetaminophen was taken, an IEP or Early Intervention services, whether a lawyer already represents this claim, and the best time to call. We also store the consent checkbox, the time, your IP address, your browser user-agent, the page address, and the TrustedForm certificate URL and ping URL if the TrustedForm script provides them. We do not ask for a free-text medical story, Social Security number, or account password. There are no user accounts.

Why

We use the answers to screen an inquiry and, when the screening fits, to share it with participating attorneys so they can decide whether to contact you. Consent text, the consent version, the time, the IP address, and the user-agent are stored with the inquiry so there is a record of what you agreed to.

TrustedForm

The home page and the intake page load a script from ActiveProspect's TrustedForm service (api.trustedform.com). That script can record a certificate of the form session and write a certificate URL into a hidden field. We store the URL. We do not turn on a paid certificate check unless the operator sets that flag. The vendor's script cannot be pinned with a fixed integrity hash because the vendor serves it dynamically. Our own scripts and fonts are hosted on this site.

Sharing

Outside delivery is off unless an operator turns a flag on. Those optional paths are a lead buyer, a customer-relationship tool, an intake validator, and a private CRM. Each stays off when its flag is off, and this site then keeps the inquiry only in the local store. We do not sell the form as a public marketing list. We may share an inquiry with a participating lawyer and with a vendor that lawyer uses to receive it, and only for that review.

How long

The local store keeps the inquiry so a duplicate submit in a short window is recognized and so the consent record is not lost. On the hosted app, that store lives on the app disk unless an operator sets a path. It is not a separate paid database. Do not send secrets in the form.

Your choices

You can refuse the form. If you already consented to calls or texts, reply STOP to a text or tell the caller to stop. Message frequency varies. Message and data rates may apply. Consent is not a condition of any purchase.
